Family To-Do List
Family To-Do List
- Course Selection
- It is essential that students and a parent/guardian attend the course selection appointment. You will meet with a North High Counselor to go over recommendations made by 8th-grade teachers, select electives, and briefly discuss any transition issues. The appointment is only 7-8 minutes, but feel free to contact the counselor after registration to discuss issues further. Students will also take their ID picture and have a chance to purchase DGN spirit wear.
- Enrollment
- More information on enrollment can be found on the District 99 website.
- Two Required Health Forms
- You must provide both a Certificate of Child Health Examination (physical) form and Proof of School Dental Examination form, dated on or after August 18, 2025, by June 1, 2026.
- The School Code of Illinois requires that each student entering 9th grade must have a physical examination on the Certificate of Child Health Examination Form. (Must be completed by a licensed physician, an advanced practice nurse or physician’s assistant. A physical examination must include: height, weight, blood pressure, BMI, diabetic screening, signatures and dates in order to be considered in compliance. Student records must show proof of immunization series according to the guidelines of the Illinois Department of Public Health; which include: Diphtheria, Pertussis, Tetanus, Polio, Measles (Rubeola), Mumps, Rubella, Hepatitis B, Varicella, Tdap and Meningococcal.) NOTE: The Health History section of the physical examination form (at the top of page one) MUST be completed, signed and dated by the parent/guardian of the child.
- A Completed Dental Exam is also required for all incoming 9th graders.
- Please complete both forms as soon as possible to prevent delays in registration and in your student receiving their class schedule. Forms must be submitted by June 1, 2026 via email, fax or U.S. mail to: North-Nurse@csd99.org or FAX: 630-795-8399
- 9th grade students will be excluded on the first day of school, and every day thereafter, until complete immunization records, physical examination and parent/guardian health history have been received and approved by Health Services.
- Persons utilizing either of the two allowable state exemptions for immunizations (medical contraindication or religious objection) MUST have their doctor complete the required exemption form in addition to the physical form.
- You must provide both a Certificate of Child Health Examination (physical) form and Proof of School Dental Examination form, dated on or after August 18, 2025, by June 1, 2026.

Future Trojan Night
Future Trojan Night
Monday, January 26, 2026
Future Trojan Night is designed to help prepare students for their registration meeting with their counselor, and also will help students and parents learn about electives students might take after their 9th-grade year. Students and parents are strongly encouraged to attend sessions during all three time slots as this is the only opportunity to hear directly from teachers about electives across departments. This information will help students with selecting electives not only in their 9th grade year, but throughout high school.

How do I register my student for an athletic team or extracurricular activity?
Families use Infinite Campus to register for clubs and activities. They will use Snap to register for extracurricular athletic teams.
Where is my bus stop, and what time will the bus pick me up?
The Board of Education provides bus transportation for students who live more than 1.5 miles from school. First Student provides this service for us. Specific bus routes operate in the morning and afternoon.
Bus routes are published shortly before the first day of school; we email routes to all families and also post them to our website. To locate your bus stop, look for the route with stops nearest to your home. Find the nearest intersection listed. Report to the bus stop at least 5 minutes before pick-up time listed. If a bus is full, the driver will radio the bus company for another bus to pick you up.
