Transfer to DGN

In order to attend our schools, you must live within the D99 attendance area.

Check Boundaries

Gather required residency documents:

Category A (ONE document showing a current DGN address)

• Real estate tax bill
• Mortgage statement/Closing contract
• Signed lease (with a contact phone number for the leasing agent)
• Agreement of sale

If the student’s family is not the owner/lessee of the District 99 address, additional forms may be obtained from the registrar and are required.

Category B (TWO documents showing a current DGN address)

• Driver’s license
• Home/apartment insurance
• Auto insurance
• Cell Phone / Internet bill
• Utility bill within the past 2 months 
• Current dated bank statement or current dated payroll stub

Other Required Documents

• Certified Birth Certificate (not a copy) OR student's passport, if birth certificate is unavailable
• Parent/Guardian Photo ID

All documents must be brought in and shown to the DGN registrar. 

Thank you for taking the time to gather these important documents for the enrollment process.  Having these ready and available will help you complete the enrollment process for your student. 

Please note: If you are in temporary residence or are homeless as defined by The McKinney-Vento Act (living in a shelter, hotel, or sharing housing with a relative), contact DGN's McKinney-Vento liaison after you have completed the Transfer Form to learn about other supports available for your student.

Documents Required from Previous School

It is the parent/guardian responsibility to request transfer records

• Illinois State Board of Education (ISBE) Student Transfer Form, or a "letter of good standing" on school letterhead from private or out-of-state schools.
• An unofficial transcript for current high school students.
  • 7th and 8th grade report cards and course recommendations for incoming 9th grade students.
• A State of Illinois Certificate of Child Health Examination including immunizations with the physician's signature and address, as well as dental and eye examinations. A printout of immunizations does not meet this requirement.
  • Incoming 9th grade students must present a valid physical examination form before the first day of school.
  • Out-of-state transfers: immunizations must be provided, and an Illinois physical will be required within a certain timeframe, along with dental and vision exams.
• Current class schedule and grades in progress if transfer is occurring during the school year.
• Current IEP or 504 Plan for any student receiving specialized services at their previous school.

Additional Documentation as Needed

Specific situations may require further documentation to determine the residential custodian of the student based on care or custody agreements or situations regarding residency.  Please be prepared to share such documents, if requested.  

All required health forms must be returned to the Health Office before a student begins classes at Downers Grove North. Parents/guardians will need to review and return the required health documents and complete these forms:  

• Child Health Examination Form  
• Eye Examination Form  
• Dental Examination Form  
• Medication Permission Form  
• Asthma Action Plan (if applicable)  
• Anaphylaxis Action Plan (if applicable)

Once all forms are completed and documents are collected, the Registrar will contact you to schedule an enrollment appointment.

Please note, that if any documentation is missing, it halts the transfer process.

Before your first day at DGN, your DGN School Counselor will help you select your courses, create your schedule, and share other important information to support your transfer to DGN. You may need to participate in academic assessments, such as English and Math so that we can make sure you are placed in the correct classes. Your DGN School Counselor will work with our department chairs to ensure you are enrolled in courses that meet graduation requirements.

The best way to experience all that DGN has to offer is to expand your participation beyond academics and become involved in a club, activity or athletic program!
